A Power of Attorney for Healthcare allows you to name someone you trust to make medical decisions for you when you can't.


 The purpose of this Power of Attorney is to give your designated 

"agent" broad powers to make health care decisions for you, 

including the power to require, consent to, or withdraw treatment 

for any physical or mental condition, and to admit you or discharge 

you from any hospital, home, or other institution. 


You may name successor agents under this form, but you may

 not name co-agents.


This release authority in a Power of Attorney for Healthcare applies to any information governed by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and regulations thereunder. 


 It allows the person named to act as your agent to serve as your "personal representative" as that term is defined under HIPAA 

and regulations thereunder.

A Power of Attorney for Property allows you to name someone you trust to handle your financial affairs for you when you can't. 


The purpose of the Power of Attorney for Property is to give your designated "agent" broad powers to handle your financial affairs, 

which may include the power to pledge, sell, or dispose of any of 

your real or personal property, even without your consent or any 

advance notice to you. 


Many times our client's choose to have these powers become effective upon a doctor's written determination that you are unable to handle your own financial affairs.  


You may name successor agents, but you may not name co-agents.


The Power of Attorney for Property may be amended or revoked

 by you at any time.
